Regional Community Health Needs Assessment (rCHNA)

The Challenge

Through the Patient Protection and Affordable Care Act (PPACA), non-profit hospitals are required to complete a Community Health Needs Assessment and implementation plan to inform their community benefit and community building activities every three years. This assessment is an important opportunity to hear directly from community residents and community organizations. In southeastern Pennsylvania, many different health systems frequently assessing communities have the potential to increase community burden and fatigue.

What We Do

Our Solution

HCIF coordinates the regional, collaborative Community Health Needs Assessment (rCHNA) on behalf of nine area health systems. The rCHNA assesses population health and social needs indicators for geographic communities, and engages community voices in one streamlined process to gain their perspective on community assets, needs, and proposed solutions.
