CDC 1817:

Heart Disease Prevention

CDC 1817: Heart Disease Prevention is a five-year initiative funded by a grant awarded to the Philadelphia Department of Public Health (PDPH) in 2018 by the Centers for Disease Control and Prevention (CDC). CDC 1817 seeks to improve blood pressure and cholesterol control in the city, especially among Philadelphians with Medicaid coverage and residents of areas of the city shown to have poor cardiovascular outcomes. 

HCIF’s activities include:

  • Convening collaborative meetings of participating health systems and health centers

  • Advancing self-measured blood pressure monitoring

 

  • Facilitating referrals to community resources that support heart health
